Applying for Crime Victims' Compensation If you or a family member has suffered personal injury as a result of a violent crime, trying to stop a person committing a crime, trying to help a law enforcement officer, trying to help a victim of a crime or, witnessing a violent crime, you can complete the following form to file a claim for compensation from the South Dakota Crime Victims' Compensation Program.
Please follow all directions when filling out the application. Provide as much information as you can regarding the crime and the crime-related expenses for which you are seeking assistance. You need to complete the W9 form only if you are seeking reimbursement for amounts that you have paid or for lost wages.
